Free structured diabetes education to help you live well with type 2 diabetes

In Lincolnshire, there are 56,000 people affected with type 2 diabetes, and the rate is rising. 

Type 2 diabetes is a serious condition and is lifelong. If left uncontrolled, it can cause problems with your eyes, feet, kidneys, and heart. 

However, with the right treatment and care you can control your diabetes and even put it into remission. Remission means your blood sugar levels are below the diabetes range, and you don’t need to take diabetes medication. 

Empowering you with the knowledge to live well with diabetes

It’s important that people with type 2 diabetes get the information they need to best support their health. 

If you have type 2 diabetes and live in Lincolnshire, you can access a new structured diabetes education programme. 

Provided to the NHS, EMPOWER is designed to help you understand what type 2 diabetes is, the effect it has on your body and how to make small, achievable changes to the food you eat and your everyday life. 

Key information:

  • Free to attend
  • Four hours long
  • Small group education, available face-to-face or via Microsoft teams
  • You can either self-refer or ask your GP to refer you.

Learning how to manage your diabetes will help you improve your quality of life and reduce the risk of diabetes-related complications.
