NHS Lincolnshire CCG - Dementia Services Review Survey
NHS Lincolnshire Clinical Commissioning Group and Lincolnshire County Council are currently reviewing the services available for people with dementia and their carers across the county. We are trying to build a picture of how these services work currently and make improvements as required. In order to support this review, we are keen to hear from people with dementia, their families and carers regarding their views and experience of these services, what works well and what is missing or could be improved.
A short survey has been produced which can be accessed here.
When completing this survey, please consider your whole journey from diagnosis and support to live with the condition, through to end of life care if you have experience of this. Think of any difficulties you have experienced accessing information, advice or support, or anything that has really helped you in managing any part of your journey – this might include your GP or other practice staff, specialist dementia services such as memory assessment clinics, mental health teams or Admiral nurses, hospital or care home staff or community support groups or activities.
The deadline for completing the survey is midday Tuesday 31st August 2021.
